Zackary Vankeulen, director of emergency services at OSF Saint Paul, and his team were finalists in the OSF Trailblazer Challenge with their idea to improve the hospital discharge process by expanding the abilities of OSF MyChart to make the "after-visit summary" more useful to patients.
